About the Position

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time provider. As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to work with patients in a variety of settings, including home visits, skilled facilities, and telehealth visits.

Responsibilities
  • Conduct comprehensive health assessments of patients in their homes and communities
  • Develop and implement individualized care plans to address the unique needs of each patient
  • Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and outcomes
  • Document patient information and outcomes in a timely and accurate manner
Requirements
  • Masters degree in nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent communication and interpersonal skills
