Job Summary

UPMC is seeking a skilled Physical Therapist to join our Home Health team in Venango County. As a Physical Therapist, you will provide professional physical therapy services in the home and community setting, including assessment, intervention, treatment, and instruction to individuals of all ages to maximize functional abilities and ensure optimal care.

Responsibilities

  • Evaluate, plan, and treat patients in an effective Physical Therapy Program, including the use of modalities and assistive devices within the Standards of Physical Therapy Practice.
  • Identify, develop, and participate in process improvement opportunities within the Agency's Home Health or Early Intervention programs to enhance the quality of services provided.
  • Assign and supervise the Certified Physical Therapy Assistant, if applicable.
  • Attend compliance training and adhere to the organization's standards of conduct, policies, and procedures.
  • Perform duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC towards patients, visitors, staff, peers, physicians, and other departments within the organization.
  • Maintain and complete all required documentation, orders, Point of Care, or written, promptly and accurately within Agency and regulatory standards.
  • Provide physical therapy services and participate in the development of a physical therapy plan of treatment, including modalities, frequency, and treatment in collaboration with the referring physician for patients from birth to geriatrics to assist patients in obtaining maximum functional independence within the standards of physical therapy.

Requirements

  • Graduate of a Physical Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association; or eligible for the National Registration Examination of the American Physical Therapy Association.
