As a Case Manager - Service Coordinator, you will be responsible for providing service coordination to individuals with intellectual disabilities, ensuring they receive the necessary support and resources to achieve their goals.

Key Responsibilities:

  • Service Coordination: Develop and implement individualized service plans to meet the unique needs of each client.
  • Assessment and Monitoring: Conduct regular assessments to identify client needs and monitor progress towards goals.
  • Referral and Linkage: Establish and maintain relationships with community resources to ensure clients receive necessary services and support.
  • Documentation and Reporting: Maintain accurate and timely records of client interactions, progress, and outcomes.

Requirements:

  • Education: Bachelor's degree in a social, behavioral, or human services field from an accredited university.
  • Experience: One year of experience working directly with individuals with intellectual disabilities.
  • Certifications: Qualified Intellectual Disabilities Professional (QIDP) certification.
  • Other: Valid Texas driver's license and insurability.
