About the Role

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team as a Clinical Care Coordinator. This is an excellent opportunity for a motivated and compassionate healthcare professional to make a real difference in the lives of our patients.

Key Responsibilities:

  • Providing comprehensive in-home assessments for patients and collaborating with the primary care team to develop effective care plans.
  • Reviewing patient symptoms, identifying diagnoses, and implementing treatment plans that prioritize patient-centered care.
  • Performing physical examinations, reviewing medication regimens, and conducting vital checks, lab draws, wound care, and depression screenings as needed.
  • Communicating assessment findings to inform the PCP of potential gaps in care and driving high-quality metrics and closure rates.
  • Fostering strong relationships with patients, families, and caregivers to deliver personalized care that addresses their unique needs.
  • Thinking critically to address complex patient needs, providing resources and follow-up care to improve stability and outcomes.
  • Educating patients on disease management, medication adherence, and self-care strategies to promote healthy behaviors.
  • Maintaining the highest level of confidentiality and adhering to HIPAA regulations at all times.
  • Traveling frequently to provide care to patients in their homes, often in diverse settings and weather conditions.

Requirements:

  • A minimum of 2+ years of clinical experience as an NP/PA in primary care or home care settings.
  • Working knowledge of chronic care management, various medication options, and evidence-based practices.
  • Experience working in Family Medicine, Internal Medicine, Gerontology, Palliative Care, or related fields.
  • Unrestricted NP or PA license in the state of IN, current DEA licensure/prescriptive authority, and certification in BLS or ACLS.

Benefits:

  • A competitive salary ranging from $110,000 to $140,000 per year, depending on experience.
