Role Summary

As a Case Manager, you will play a vital role in providing holistic, patient-centered care that addresses both immediate and long-term needs. You will work closely with healthcare professionals to identify, coordinate, and provide appropriate levels of care under the direct supervision of an RN or MD.

Key Responsibilities

  • Perform clinical operations and medical management activities across the continuum of care, including case management, coordination of care, and medical management consulting.
  • Provide health education, coaching, and treatment decision support for members.
  • Travel approximately 50% of the time to meet with members in the hospital.

Requirements

  • High School diploma or GED or equivalent work experience.
  • Unrestricted LPN or LVN license in the State where the position will be performed.
  • 3+ years case management or acute care experience.
  • Basic computer proficiency (Microsoft Word, Outlook, and internet).
  • Ability to travel 30-50% of the time to local hospitals on a daily basis.

Preferred Qualifications

  • Experience with EMR (Electronic Medical Records).
  • Inpatient Hospital Case management experience.
  • Bilingual - English/Spanish.
