Job SummaryWe are seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN at Ambercare, you will provide direct patient care, implement care plans, and educate patients and families on self-care techniques. You will work collaboratively with our interdisciplinary team to ensure seamless care delivery.
Responsibilities- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Implement care plans initiated by registered nurses, ensuring accurate and timely documentation.
- Assess and educate patients and families on self-care techniques, disease management, and medication adherence.
- Participate in coordination of home health services, reporting identified needs to the registered nurse and Clinical Supervisor.
- Use and prepare equipment and supplies effectively, adhering to aseptic technique.
- Participate in personal and professional growth and development, staying up-to-date with industry best practices.
- Graduate of an accredited practical nurse or vocational nursing program.
- Three years of nursing experience, with community/home health or medical/surgical experience preferred.
- Current licensure as a licensed practical nurse or licensed vocational nurse in the state.
- Possess and maintain valid CPR certification.
- Reliable transportation, current driver's license, and appropriate automobile insurance.
