Position Overview
The Clinical Liaison, reporting to the Director of Patient Care Services, employs the nursing process to design, execute, and assess outcomes for specific patient demographics.
This role requires a diverse skill set to educate, consult, and assist in managing patients within a defined population, aiming for optimal quality and outcomes while maintaining cost-effectiveness.
The Clinical Liaison collaborates with a multidisciplinary patient care team, including physicians, nursing staff, and other healthcare professionals, to identify opportunities for enhancing patient care and outcomes while reducing costs based on current research.
This position supports the mission, vision, and values of Atrium Health and contributes to achieving its objectives.
Qualifications
Required Education
Graduate of an accredited Registered Nursing program.
Preferred Education
Bachelor of Science in Nursing.
Licensure/Certification Requirements
Current Registered Nurse license in the state of practice.
Preferred Licensure/Certification
Membership in a relevant professional organization.
Specialty certification.
Required Experience
A minimum of ten years in patient care for the designated population.
Preferred Experience

Key Responsibilities
Clinical Expertise:
Coordinates and delivers direct patient care as necessary, collaborating with medical staff to promote health and prevent illness.
Responds to requests from healthcare team members regarding complex patient needs.Adjusts care plans to meet individual patient requirements.
Engages patients and their families in goal-setting and communicates effectively about care plans and discharge objectives.
Participates in discharge planning.Collaborates regularly with physicians regarding patient progress and needs.
Conducts rounds with providers to discuss patient status.
Works closely with Directors to evaluate supplies and equipment supporting service lines.
Partners with utilization managers to identify service line customers, assess needs, plan interventions, implement activities, and evaluate outcomes that exceed expectations.
Leads team huddles to anticipate and address discharge barriers.Escalates discharge barriers as necessary.
Ensures the care team is accountable for meeting anticipated discharge dates.
Performs additional related duties as assigned.
Research Participation:
Facilitates the development of clinical pathways based on scientific research.
Monitors performance data and significant outcomes to evaluate current clinical practices.
Leads quality improvement initiatives for targeted patient populations using a multidisciplinary approach.
Contributes to clinical research studies relevant to designated patient groups.
Educational Resource Development:
Identifies educational needs and collaborates with the patient care team to create programs and materials for patients, families, and the community.
Works with staff to enhance necessary skills and documentation tools.Enhances knowledge through participation in continuing education programs, focusing on age-related specialties and leadership.
Collaborates with Directors to identify staff learning needs and provide instructional support.Engages with relevant customer groups in strategic educational planning for the organization and community.
Administrative Responsibilities:
Promotes the mission, objectives, and strategic planning of Atrium Health.
Collaborates with the healthcare team to enhance efficiency, cost-effectiveness, and clinical outcomes.
Assists in evaluating and revising policies and procedures in conjunction with the healthcare team.
Age-Specific Care:
Communicates effectively with patients and families across all age groups.
Provides age-appropriate environments, education, and procedures within the specialty area.

Frequent exposure to communicable diseases, bodily fluids, toxic substances, and other clinical environment risks.
Physical Demands:
Full range of motion including patient handling, manual dexterity, and coordination. Standing and walking for extended periods. Occasionally lifts weights up to 50 pounds.
Ability to perceive letters, numbers, and symbols with normal vision and hearing to respond to alerts and alarms.
