About the Role:
We are seeking a highly skilled Clinical Pharmacist to join our team at Virginia Hospital Center. As a Clinical Pharmacist, you will play a critical role in ensuring the safe and effective use of medications across our healthcare system.
Key Responsibilities:
- Develop and implement medication management plans for patients of all ages, utilizing evidence-based guidelines and standards.
- Provide pharmaceutical care and advanced clinical support to nursing staff, physicians, and patients, ensuring seamless coordination of care.
- Collaborate with interdisciplinary teams to optimize medication use, reduce adverse events, and improve patient outcomes.
- Stay up-to-date with the latest medication trends, guidelines, and regulations, applying this knowledge to inform practice and improve patient care.
- Conduct medication use evaluations, identifying areas for improvement and implementing changes to optimize medication use.
- Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures related to medication management.
Requirements:
- Graduate degree from an accredited pharmacy program (ACPE).
- Doctorate in Pharmacy or Pharmacy Residency preferred.
- Minimum two years of experience in a healthcare setting, with a focus on pharmaceutical care or medication management.
- Licensure as a Pharmacist by the Virginia Department of Health Professions.
