The Licensed Practical Nurse (LPN) role at Wexford Health is a critical part of our patient care team. Under the direct supervision of the designated Wexford Health manager and clinical supervision from the Director of Nursing, Registered Nurses, Medical Director, and/or IDOC personnel as appropriate, the LPN is responsible for assisting in the delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation.
The LPN directs and guides patient teaching activities commensurate with education and demonstrated competencies. Utilizes electronic medical record system as applicable throughout the scope of duties and responsibilities.
The LPN is responsible for:
- Performing non-professional nursing care duties in the assessment of the physical, psychological, and social dimensions of patients in the Health Care Unit and, as necessary, in the housing units.
- Assisting in planning individual treatment programs by utilizing available resources in planning care and consulting with registered nurses, physicians, and correctional officers.
- Assisting in implementing individual treatment programs in the Health Care Unit and, as necessary, in the housing unit.
- Assessing personal and learning development needs.
- Planning and implementing programs to meet identified needs through in-service attendance and other formats and informal means.
- Demonstrating knowledge of, and performing nursing policies and procedures.
- Supporting the philosophy, objectives, and goals of the nursing department.
The LPN must possess:
- Current licensure as an LPN in the state of Illinois.
- Current CPR certification accepted by the American Red Cross, American Heart Association, or American Safety and Health Institute.
- Graduation from an accredited School of Nursing.
- Preferred experience: one year of clinical experience and prior experience with the use of electronic medical record systems.
We offer a rewarding career opportunity with a competitive salary of $34.00 per hour, plus an additional $4.00 for evening shifts and $3.00 for night shifts, totaling $41.00 per hour. Please note that the location for this role is Dixon, IL.
