Job Title: Physical Therapist (PT) Outpatient

Centra Healthcare Allied is seeking a skilled Physical Therapist (PT) to join our team in Eden Prairie, Minnesota.

Job Summary:

We are looking for a highly motivated and experienced Physical Therapist (PT) to work in our Outpatient department. The ideal candidate will have a strong background in physical therapy and excellent communication skills.

Responsibilities:
  • Provide high-quality physical therapy services to patients in an outpatient setting
  • Develop and implement individualized treatment plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Document patient progress and maintain accurate records
Requirements:
  • 1 year of recent experience as a Physical Therapist (PT) specializing in Outpatient
  • Current Physical Therapist (PT) license within the state of practice
