Key Responsibilities- Conduct comprehensive assessments of patients' needs and develop personalized treatment plans to improve mobility, reduce pain, and restore function.
- Deliver physical therapy services to patients in their homes, adapting treatment plans to suit individual needs and home environments.
- Collaborate with physicians and other healthcare professionals to ensure seamless care and effective communication.
- Teach patients and their families how to perform exercises and use adaptive devices to promote independence and self-care.
- Document patient progress and maintain accurate records to ensure high-quality care and compliance with regulatory requirements.
- Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
- Licensed or registered by the State.
- At least one year of institutional experience.
- Home care experience and supervisory experience preferred.
- Ability to drive and access clients' homes, which may not be routinely wheelchair accessible.
