NYC Health + Hospitals/Community Care is seeking a skilled Home Health Nurse to provide high-quality care to patients in their homes. As a Home Health Nurse, you will assess, plan, implement, and evaluate patient care plans, providing direct skilled nursing care and observing and instructing patients in their place of residence.
Responsibilities- Assess patient needs and develop individualized care plans
- Provide direct skilled nursing care, including wound care, medication management, and vital sign monitoring
- Observe and instruct patients in their place of residence, promoting independence and self-care
- Coordinate services of other caregivers, as needed
- Document patient progress and communicate with healthcare team members
- Current certification in Basic Life Support procedures, including CPR
- Minimum two (2) years of full-time nursing experience, preferably in Homecare, Long-term Care, or Hospital setting
- Knowledge of Federal, State, and local regulations and JCAHO standards for Home Care
- Excellent verbal, written, organizational, and analytical skills
- Baccalaureate Degree in Nursing from an approved program
- Experience using various computer software applications, including Word, Excel, Access, and EPIC H2O
