Responsibilities- Provide direct patient care to residents in a Long Term Care setting, including administering medications, wound care, and vital sign monitoring.
- Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each resident.
- Conduct thorough assessments of residents' physical and emotional needs, and document findings in a timely and accurate manner.
- Participate in interdisciplinary team meetings to discuss resident care and develop strategies for improving patient outcomes.
- Maintain accurate and up-to-date records of resident care, including medication administration, treatment plans, and progress notes.
- Current LPN licensure in the state of Missouri or a compact state.
- At least 2 years of experience in a Long Term Care setting.
- AHA BLS certification.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Competitive hourly rate of $38.00.
