SLP Care Coordination Specialist

We are seeking a skilled SLP Care Coordination Specialist to join our team at EmpowerMe Wellness. In this role, you will provide speech-language pathology and wellness services to residents of senior living communities while supporting care coordination efforts.

You will conduct assessments, treatment planning, and therapeutic interventions as part of a multidisciplinary team. Additionally, you will communicate with patients' providers and caregivers, and assist patients with attending telehealth visits.

This is a fantastic opportunity for a motivated Speech-Language Pathologist who wants to make a difference in the lives of seniors. We offer a competitive salary range of $100,000 - $130,000 per year, depending on experience.

Key Responsibilities:

  • Provide speech-language pathology and wellness services to older adults.
  • Conduct assessments, treatment planning, and therapeutic interventions as part of a multidisciplinary team.
  • Support care coordination efforts by conducting regular wellness checkpoint visits with patients, communicating with patients' providers and caregivers, and assisting patients with attending telehealth visits.
  • Maintain accurate and detailed records of patient interactions.

Requirements:

  • Completion of an accredited Speech-Language Pathologist program (New Grads Welcome).
  • Active ASHA certification and current state licensure in SLP.
  • Excellent communication and interpersonal skills.
  • Able to work effectively in a fast-paced environment.
