Job Summary

As a key member of our team, you will be responsible for delivering exceptional care to our patients, working closely with our registered nurses to ensure seamless care coordination.

Responsibilities
  • Assessment and Planning: Utilize the nursing process to assess patient needs, develop individualized care plans, and implement evidence-based interventions to achieve optimal patient outcomes.
  • Patient Care: Provide direct patient care, including vital sign monitoring, medication administration, and wound care, under the supervision of a registered nurse.
  • Communication: Effectively communicate with patients, families, and healthcare team members to ensure seamless care coordination and address any concerns or questions.
  • Documentation: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes, in accordance with company policies and regulatory requirements.
Qualifications
  • Education: Current Licensed Vocational Nurse (LVN) license in the state of practice.
  • Experience: One year of prior LVN experience, preferably in a private duty or home healthcare setting.
  • Certifications: Current BLS certification and PPD or chest X-ray, as required by the facility.
Skills and Abilities
  • Enteral Feeds: Ability to administer enteral nutrition and manage related equipment.
  • Tracheostomy Care: Knowledge of tracheostomy care and management, including suctioning and tube changes.
  • Ventilator Management: Experience with ventilator management, including setting and adjusting ventilator settings.
