Family Wellbeing Facilitator

We are seeking a highly motivated and experienced Family Wellbeing Facilitator to join our team at Child & Family Focus, Inc. in Bucks County.
This is an exciting opportunity to work with children and families who require support and guidance to improve their mental health wellbeing.
The successful candidate will be responsible for facilitating High Fidelity Wraparound processes to empower families and youth to have their voices heard through individualized planning.

About the Position
The Family Wellbeing Facilitator will work closely with the youth and their family to engage them in the High Fidelity Family Teams process.
Key responsibilities include:
- Facilitating team meetings to identify and express needs.
- Collaborating with service providers and natural supports.
- Developing and implementing individualized plans.
- Monitoring plan implementation and communicating with the HFW Team.
- Teaching High Fidelity skills to youth, family, and natural supports.
Requirements
To be successful in this role, you will need:
- A Bachelor's Degree in Human Services or a related field.
- Two or more years of experience in providing services for children and families.
- Experience managing teams in the mental health or child welfare sectors.
- Ability to work 2-3 evenings per week (5:00-7:30pm).
Salary and Benefits
We offer a competitive salary of $38,528 per year ($19 per hour) based on a full-time schedule.
