Job Summary and Qualifications
As a Clinical Nurse Case Manager, you will play a vital role in promoting patient-centered care by coordinating the patient's care plan, managing their length of stay, and ensuring effective resource management. You will work collaboratively with a multidisciplinary team to develop safe and appropriate discharge plans.
Your responsibilities will include:
- Facilitating care transitions and progression using established criteria.
- Coordinating activities that enhance quality outcomes and patient throughput while balancing optimal care with resource utilization.
- Providing case management services for both inpatient and observation patients as assigned.
- Identifying patients at risk for adverse outcomes during transitions between care levels.
- Conducting comprehensive assessments of psychosocial, medical, and discharge needs of patients and their families.
To be successful in this role, you should have:
- A minimum of 2 years of experience in case management or 3 years in clinical nursing.
- Preferred experience with InterQual.
- An Associate Degree in Nursing or Nursing Diploma; a Bachelor's Degree in Nursing is preferred.
- Current licensure as a Registered Nurse in the state of practice.
