The Registered Nurse (RN) Case Manager is responsible for coordinating client care and serves as the primary contact for all disciplines involved in patient care. This role oversees the frequency of visits throughout the care episode, consults with physicians as necessary, and provides detailed updates on patient care. Essential duties include accurate OASIS data collection, maintaining current medication profiles, ensuring timely reporting of lab values to physicians, and participating in weekly case conferences and monthly case manager meetings.
QualificationsTo qualify for this position, Registered Nurses (RNs) must meet the following criteria:
- Possess a valid Registered Nurse (RN) license in the state of employment
- Have a minimum of one year of clinical experience (preferred)
- Demonstrate proficiency in current nursing practices
- Hold a valid state driver's license and automobile liability insurance
