Patient Advocacy Liaison

We are seeking a skilled Patient Advocacy Liaison to join our team at St. Barnabas.
About the Role:
- The ideal candidate will have a minimum of two years' experience in the healthcare industry and possess advanced understanding of medical terminology and administration processes.
- They will be proficient in information management programs and MS Office, with outstanding communication and interpersonal abilities.
- A strong attention to detail and excellent organizational skills are essential for this role.
- Demonstrated proficiency in English and Spanish is preferred, as well as the ability to work effectively in a team.
Key Responsibilities:
- Serves as a linkage between patients and primary care providers to improve the coordination of care.
- Collaborates with physicians, clinic staff, and other interdisciplinary staff to identify care needs and connect patients with appropriate services.
- Coordinates care for patients who have gaps in care or quality flags, to close these gaps by outreaching, engaging, and scheduling patients.
- Coordinates patient scheduling, cancellations, and re-scheduling.
- Communicates with patients through telephone, mail, MyChart, and text messages where appropriate, to increase patient attendance.
- Answers the telephone in a timely manner, politely, and in a professional manner.
- Meets with patients, primary care, and BH providers, and other staff as needed to coordinate care.
- Encourages patient self-management with a focus on the achievement of care plan goals.
- Documents and maintains patient-specific records.
- Maintains accurate daily logs for statistical reporting purposes and for weekly reviews of productivity.
- Meets leadership when necessary to discuss various concerns involving the functions and performance of the Ambulatory Care Department.
- Completes mandatory hospital in-services via Healthstream.
- Logs into E-Mail, and any other necessary program, daily.
- Performs any other duties as assigned.
Compensation: The estimated salary for this position is $65,000 - $85,000 per year, depending on experience.
