We are seeking a skilled and compassionate Home Health Nurse to join our team at connectRN Inc. As a Home Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Conduct comprehensive patient assessments and develop individualized plans of care in collaboration with the multidisciplinary care team.
  • Provide patient episode management per protocol and maintain continuity of patient care by communicating effectively with all team members.
  • Educate patients, caregivers, and families on aftercare and ongoing care.
  • Perform vital signs and patient assessments during visits.
Requirements
  • Current and unencumbered RN or LPN license.
  • 1+ year(s) of Certified Home Health experience as an RN or LPN.
  • OASIS experience (RNs only).
  • Strong assessment, critical thinking, and communication skills.
  • Current CPR certification.
  • Comfortability using technical tools and software/EMR for visit documentation.
  • Reliable transportation, current driver's license, and car insurance coverage.
  • Proof of Immunization/Exemption for Flu and COVID-19.
  • TB Test or Chest X-Ray.
