Job Summary:We are looking for a seasoned Registered Nurse with expertise in Case Management to fill a travel nursing position in Langhorne, Pennsylvania. As a Travel Nurse Manager RN - Case Management, you will be responsible for assessing, planning, evaluating, and coordinating patient care, including discharge planning and utilization needs.
Key Responsibilities:- Assess patient needs and develop individualized care plans to ensure optimal patient outcomes.
- Collaborate with the interprofessional team to coordinate patient care and achieve goals.
- Identify and eliminate barriers to effective patient stay management and promote improvements in work processes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless continuum of care.
- Document patient assessments, interventions, and outcomes in accordance with documentation standards and policies.
- Current BLS and ACLS certifications.
- Minimum of 2 years of experience in Case Management.
- Ability to float throughout the hospital and/or multiple hospital locations.
- EPIC experience preferred.
- Cannot hold a concurrent position at a Trinity Health facility.
- Premium pay and flexible scheduling.
