Hospice Registered Nurse Job Description

About the Role:

The Hospice Registered Nurse is a vital member of our interdisciplinary team, responsible for evaluating clients' physical, emotional, psychosocial, and spiritual needs. This role requires a skilled nurse to develop and implement care plans, ensuring the highest quality of care for our clients.

Key Responsibilities:

  • Coordinate client care and services, including social workers, chaplains, in-home aides, physicians, and volunteers.
  • Assess clients and families within 24 hours of admission to Hospice.
  • Provide skilled nursing care as determined by the IDG plan of care.
  • Offer education to clients, families, and others on care techniques specific to each client.
  • Advocate for clients and families by maintaining regular communication with physicians and other resources as needed.
  • Participate in bereavement support for clients and families.
  • Document and ensure proper use of medications, DME, and supplies.
  • Attend weekly meetings and IDG meetings.
  • Develop and leave care plans in homes for other staff members and contract staff personnel.
  • Monitor recertification and reauthorization processes.
  • Accurately document and follow through with revocation processes.
  • Report regularly to the Hospice Administrator, providing client updates and status.
  • Maintain confidentiality in all aspects of the job.
  • Promote personal safety and a safe environment for clients and colleagues.
  • May complete In-Home Aide annual performance appraisals and assist with yearly competency testing.
  • Participate in on-call rotations.
  • Participate in performance improvement initiatives as requested.
  • May participate in orientation of new staff.
  • May act as a preceptor for new staff.
  • Represent the nursing profession in a positive manner.
  • May participate in community educational activities as needed.
  • Promote and participate in professional development for nursing and palliative care.
  • Perform additional duties as requested.

Working Environment:

Our Hospice Agency serves a large geographic area, including Cumberland, Robeson, Scotland, and surrounding areas.

Requirements:

  • Active NC nursing license.
  • Function as a core member of the IDG.
  • Ability to speak in front of groups.
  • Maintain high-quality customer service approach.
  • ADN required.
  • At least one year of hospice or home health nursing experience preferred.
  • Possess active listening and excellent assessment skills.
  • Working knowledge of various computer programs desired.
