Job Summary
The Social Worker is a key member of our healthcare team, responsible for assessing, counseling, and educating patients and their families to facilitate movement throughout the continuum of care. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Responsibilities
- Assess patient needs and develop individualized care plans
- Counsel patients and their families on health-related issues
- Implement and evaluate the effectiveness of care plans
- Collaborate with healthcare teams to ensure seamless care transitions
- Provide education and support to patients and their families on disease management and health promotion
Requirements
- Bachelor's degree in Social Work or a related field
- 2-4 years of experience in a healthcare setting
- NJ Licensed Clinical Social Worker
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
