Job Summary

We are seeking a compassionate and dedicated Personal Care Specialist to join our team at Ohio's Hospice. As a Personal Care Specialist, you will provide superior care and support to patients and their families, working closely with our team to ensure the highest level of care and service.

Key Responsibilities
  • Provide personal care and support to patients in their homes or facilities, promoting dignity and compassion in all interactions.
  • Assist patients with daily living activities, such as bathing, dressing, and grooming, while maintaining their independence and autonomy.
  • Work collaboratively with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.
  • Provide emotional support and companionship to patients and their families, helping to alleviate stress and anxiety.
  • Document patient care and services provided, ensuring accurate and timely reporting.
Requirements
  • High school diploma or equivalent required; one year of experience in patient care or a related field preferred.
  • Certified State Tested Nursing Assistant (STNA) certification required; certification in Hospice and Palliative Medicine (CHPNA) preferred.
  • Valid driver's license and safe driving record required; ability to drive during daytime, nighttime, or inclement weather.
  • Computer skills sufficient to properly document services and care; ability to work effectively in a team environment.
