Salary

The estimated annual salary for this position ranges from $85,000 to $110,000 based on experience and location.

Job Description

As a Hospice Nurse Practitioner, you will play a vital role in providing high-quality patient care. Your primary responsibilities will include:

  • Ensuring that the interdisciplinary care plan aligns with the patient's clinical course, continuing care needs, and covered services by monitoring diagnostic testing, treatments, and procedures as necessary.
  • Collaborating with physicians to discuss the appropriateness of resource utilization, consultations, treatment plans, estimated length of stay, and discharge plans.
  • Focusing on complex patients, frequent emergency department utilizers, chronic pain, and substance abuse patients, as well as homeless patients.
  • Working closely with acute care case managers to ensure the appropriateness of ongoing care.
  • Coordinating the transfer of patients to tertiary centers, including the transfer of patient information required for continuity of ongoing treatment and services.
Required Skills and Qualifications

To be successful in this role, you will need:

  • At least 2 years of experience in hospice care - This is a requirement.
  • Proficiency in OASIS - This is also a requirement.
  • An active Oregon license at the time of submission - No exceptions allowed.
  • Mileage reimbursement at a rate of 0.58 cents per mile.
