Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Quality Talent Group. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Deliver and coordinate quality patient care in accordance with physician orders and established care plans.
  • Continuously assess and evaluate patient condition and care needs, reporting changes to supervisors and physicians as necessary.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.
  • Participate in the development and implementation of individualized care plans, ensuring patient-centered and evidence-based care.
  • Collaborate with healthcare team members to ensure seamless care coordination and communication.
Requirements
  • Graduate of an accredited school of nursing with a current, unrestricted state license as a Licensed Nurse.
  • Current CPR certification and demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies.
  • One year of experience in pediatrics or a related field, with a strong focus on patient-centered care and communication.
Preferred Qualifications
  • Six months of recent experience as a Licensed Nurse in a clinical care setting, with a strong emphasis on home health experience.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
Physical and Environmental Demands
  • Ability to lift 50 pounds and move equipment without assistance.
  • Must be able to travel and work in a variety of environments, including homes and healthcare facilities.
  • Must be able to speak, write, read, and understand English, with a strong focus on effective communication and patient education.
