Nurse Case Manager

Interim HealthCare of New Hampshire Careers is seeking a skilled Registered Nurse Case Manager to join our team.
Job Summary:We are looking for a compassionate and experienced Registered Nurse to manage a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community.
Responsibilities:- Manage a caseload of patients, providing direct care and comprehensive nursing services
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with healthcare teams to ensure seamless care coordination
- Conduct regular assessments and evaluations to ensure patient safety and well-being
- Provide education and support to patients and their families on disease management and self-care
- Valid nursing license in the state of New Hampshire
- 2+ years of experience in skilled care
- 1+ year of experience in nursing supervision
- Recent wound care experience
- Ability to lift 50 pounds with no restrictions
- Current CPR certification
- Reliable transportation
