Role Overview:
The Clinical Care Coordinator plays a pivotal role in collaborating with various healthcare professionals, including Nurse Managers, Directors of Nursing, and physicians, to strategize, oversee, and assess patient care.

Key Responsibilities:
  • Utilize expert clinical knowledge to conduct thorough data collection and assessments in partnership with the multidisciplinary team.
  • Facilitate effective communication among healthcare professionals to enhance patient care outcomes.
  • Coordinate patient admissions, transfers, and discharges, ensuring optimal delegation and supervision of tasks among team members.
  • Guide staff on infection control practices and ensure compliance with established policies.
  • Empower patients and their families in decision-making processes regarding health and treatment.
  • Participate in quality assurance initiatives and implement corrective actions as needed.

Qualifications:
To be successful in this role, candidates should possess:
  • A Registered Nurse (RN) degree from an accredited nursing program
  • 2 to 4 years of relevant nursing experience, preferably in long-term care, medical-surgical, or inpatient psychiatry settings
  • Experience in inpatient psychiatry is preferred
