Job Summary

We are seeking a compassionate Physical Therapist to provide medical support to our patients in the comfort of their own homes. The ideal candidate will have a can-do attitude, excellent interpersonal skills, and a current state PT license.

Key Responsibilities:

  • Assess and evaluate patients' therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
  • Direct physical therapy treatment, instruct patients and families/caregivers in the use and care of therapeutic appliances, and determine priority needs for physical therapy.
  • Report to physicians patients' reactions to treatment or changes in condition, and initiate physical therapy programs.
  • Train patients in the use of prosthetic devices, identify patient and family/caregiver needs for other home health services, and refer as necessary.
  • Prepare and submit clinical and progress summaries based on the attainment of goals, and participate in discharge planning for patients.

Requirements:

  • Possess a degree from a program in physical therapy approved by an accredited organization.
  • Licensed to practice as a physical therapist within the state.
  • Two years of appropriate experience as a physical therapist, preferably in community/home health settings.
  • Demonstrate good verbal and written communication and organization skills, and possess current CPR certification.
