Job Overview


We are seeking a skilled Maternal Child Health Social Work Specialist to join our team at the University of Minnesota Masonic Children's Hospital. This is a 0.9 FTE position, offering a competitive salary of $75,000 - $85,000 per year, depending on experience, plus a $4,500 incentive.


Job Responsibilities


The successful candidate will provide a range of social work interventions to patients and families, including case management, counseling, crisis intervention, and advocacy. This will involve identifying and addressing complex psychosocial needs, recognizing rapidly changing situations, and implementing treatment and transition care plans.


Key Responsibilities



  • Provides social work interventions to patients and families, including case management, counseling, crisis intervention, and advocacy.
  • Identifies and addresses complex psychosocial needs, recognizing rapidly changing situations and quickly intervening to prevent or resolve emotional crises.
  • Plans and implements treatment and transition care plans based on psychosocial assessments, integrated into interdisciplinary care plans.
  • Arranges and coordinates a comprehensive range of community resources to meet patient and family needs.
  • Advocates for patients and families within coordinating agencies, including Child Protective Services.
  • Takes a leadership role in facilitating interdisciplinary communication regarding patients and families with complex psychosocial issues.
  • Formally and informally educates patients, families, and healthcare team members regarding the social work role, pertinent patient care issues, and population-specific information.
  • Serves as a patient advocate and liaison with physicians, families, and community to ensure continuity of care.
  • Complies with legal and regulatory laws, including mandated reporting requirements.
  • Identifies and participates in quality improvement research and program evaluation initiatives to improve healthcare services.
  • Documents patient information in electronic health records and other systems in accordance with program standards and department policies.
  • May lead interdisciplinary team debriefings in response to loss or staff moral distress.
  • Represents and participates in unit or organizational diversity, equity, and inclusion initiatives.

Reasons for Social Work Consults


Social work consults may be requested for a range of complex psychosocial needs, including mental health history or current diagnosis, substance use history or current use, positive toxicology screens for mom and/or baby, and involvement with Child Protective Services.


Qualifications



  • Master's degree in Social Work with medical experience.
  • Current Minnesota Social Work licensure at the LGSW level.

Clinical Supervision


Clinical supervision will be provided to support the development of your skills and expertise in this role.
