Job SummaryWe are seeking a Registered Physical Therapist (RPT) to offer rehabilitation services that enable individuals to reach their highest level of motor functioning and mobility. The RPT will work with consumers to develop skilled therapy goals and provide supervision to physical therapy assistants.
Responsibilities- To implement Minds Matter, LLC's philosophical principles in all aspects of service delivery.
- To review, co-sign, and provide feedback on daily notes of assistant therapists.
- To assist individuals in identifying their needs and developing appropriate compensation techniques for their brain injury.
- To perform evaluations addressing consumers' physical and mental abilities and analyze medical data to determine best methods and strategies for therapies.
- To complete documentation as necessary for an individual's plan of care.
- To integrate employment, educational, and independent living goals into therapy.
- To collaborate with consumers and other professional staff to track goal progress.
- To attend team meetings and assist consumers in goal setting.
- To provide services only as designated on each individual consumer's authorized plan of care.
- To assess and recommend adaptive equipment and/or home modifications.
- To work directly with consumers to improve skills in all aspects of their life goals.
- To document sessions with the consumer on the day of service, focusing on goal-driven, concise, factual, and clear information.
- To educate and train families and caregivers about brain injury strategies and communicate with them about goals and progress as needed.
- To maintain professional relationships with consumers, coworkers, referral agents, and community agencies.
- To promote public relations throughout the agency and community for the program.
- To serve on agency and interagency task groups as requested.
- Able to teach skills to individuals with disabilities.
- Effective supervisory and mentoring skills for therapy assistants.
- High degree of sensitivity towards all people.
- Ability to deal professionally with persons on a one-to-one basis.
- Excellent time management skills, often changing tasks without loss of efficiency or composure.
- Creativity.
- Excellent verbal and written communication skills.
- Ability to meet deadlines.
- Basic to moderate technology skills for our electronic medical record system.
The estimated annual salary range for this position is $85,000-$110,000, based on experience and qualifications.
