Caretenders Home Health is seeking a skilled Physical Therapist to join our team. As a Physical Therapist with Caretenders, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals.
Key Responsibilities
- Evaluate patients' functional status and physical therapy needs, working with the physician to develop a comprehensive plan of care.
- Develop and implement treatment plans to improve patients' mobility, endurance, and overall well-being.
- Collaborate with the healthcare team to ensure seamless care and communication.
- Provide education and support to patients and their families, empowering them to take an active role in their care.
Requirements
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Valid driver's license and vehicle insurance.
