The Clinical Pharmacist will be responsible for managing anticoagulation therapy through both in-person and virtual patient interactions, ensuring optimal patient outcomes.
Key Responsibilities:
1. Conduct targeted and comprehensive medication reviews.
2. Provide patient counseling and education on medication management.
3. Engage in disease state management and medication therapy optimization.
4. Offer drug information and address barriers to medication access and adherence.
5. Track and report productivity and quality metrics.
6. Participate in system-wide collaborative initiatives and quality improvement activities.
Schedule:
This role includes a rotating weekend staffing commitment within the inpatient environment.

Qualifications:
The ideal candidate will possess:
- Advanced professional knowledge equivalent to a Master's degree.
- Completion of a PGY-1 Pharmacy Practice Residency or a minimum of three years of direct patient care experience in a hospital setting.
- A Registered Pharmacist license in Maryland.
- Professional membership is recommended, and Board Certification is preferred.
