We are seeking a highly skilled Physical Therapist to join our team at JAG Physical Therapy. As a Physical Therapist, you will play a critical role in providing high-quality patient care and contributing to the success of our organization.
Key Responsibilities- Collect and record patient care data, including initial evaluations, daily notes, reassessments, and discharge plans.
- Develop and implement treatment plans based on patient evaluations and goals.
- Provide education and training to patients and their support systems.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Identify and address patient needs for modifications to treatment plans and goals.
- Bachelor's or Master's Degree in Physical Therapy required; Doctorate degree preferred.
- Licensed Physical Therapist in the state of practice.
- Completed state-regulated CEU requirements.
- Valid CPR license.
- Experience with EMR software.
- Excellent communication and clinical decision-making skills.
