Physical Therapist

At CHI Franciscan Health at Home, we're seeking a dedicated Weekend Physical Therapist to join our team. As a key member of our home care team, you'll provide high-quality physical therapy services to patients in their homes, empowering them to regain independence and improve their overall well-being.
Responsibilities- Develop and implement personalized home care plans to address patients' physical therapy needs
- Conduct thorough assessments and evaluations to identify areas for improvement
- Provide evidence-based physical therapy interventions to promote patient recovery and independence
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
- Document patient progress and outcomes to inform future care decisions
- Bachelor's degree from an accredited school
- Current state license
- National Provider Identifier (NPI) in accordance with Washington State Regulations
- Strong communication and time management skills
