Position Overview:

At Sage Health, our multidisciplinary care team includes Physicians, Advanced Practice Providers, Nurses, Social Workers, Case Managers, Care Coordinators, Medical Assistants, Behavioral Health Support, quality-focused Coders, Referral Coordinators, and more. This extensive support structure enables our primary care physicians to manage a reduced patient load, allowing for enhanced patient care.

Key Responsibilities:

  • Assess and treat patients in accordance with established care standards.
  • Facilitate coordination of patient services, including specialty referrals, hospital and skilled nursing facility collaboration, durable medical equipment, and home health services.
  • Actively engage as a key medical resource within the care team through daily meetings.
  • Support the Lead Physician and Center Administrator in fostering a collaborative practice environment by maintaining a professional and approachable demeanor.
  • Complete daily medical record documentation, collaborating with a quality-focused coder to ensure comprehensive records.
  • Adhere to policies and protocols set forth by Clinical Leadership.
  • Meet regularly with the Lead Physician to discuss quality of care, review outcome data, and address medical record concerns.
  • Focus on direct patient care, balancing patient-facing time with necessary administrative tasks related to patient care.

Required Qualifications:

  • Graduate of an accredited MD or DO program.
  • Possession of a current and unrestricted medical license or willingness to obtain one in the practice state; eligible and willing to secure licenses in other states as required.
  • Board Certification or eligibility for certification (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine is preferred.
  • Active and unrestricted DEA license.
  • Medicare and Medicaid Provider Numbers.
  • Demonstrated excellence in interpersonal skills and communication with colleagues and patients.
  • Fully committed to a team-based care model.
  • Willingness to learn and adapt to a value-based care environment.
  • Exceptional patient/customer service skills.
  • Basic computer proficiency, including email and electronic medical records.

Preferred Qualifications:

  • Familiarity with HEDIS/STARS metrics.
  • Understanding of CAHPS and HOS measures.
  • Experience in medically underserved or culturally diverse communities.
  • Proficiency with electronic health records.
  • Experience in Medicare Advantage Global Risk Models.
