Our nurses deliver exceptional care across the continuum.
- Make home visits to clients in designated geographic territories
- Train, coach and mentor newly hired RNs and LPNs to ensure we are delivering exceptional care to adult and geriatric patients.
- Deliver 1:1 skilled nursing, intervention, and supportive care in accordance with individual care plan, including administration of medication, wound care and treatments
- Perform client assessments, provide education to the patient and/or their family, and monitor patients' condition.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
Role Requirements:
- Current valid nursing license and graduation from a qualified nursing program
- CPR in good standing
- 5 or more years of Home Health experience
- Nurse management and education experience is preferred
