The Pharmacy Technician - Anticoagulation is responsible for supporting optimized anticoagulation patient care and service workflow. This position plays a key role in maintaining a high level of patient and care team satisfaction with exceptional customer service.Essential Duties and Responsibilities Perform as a highly dependable member of an interdisciplinary teamQuickly and accurately enter information into the EMR, review patient records and other available information to identify and work to resolve potential discrepanciesManage correspondence with providers, coordinate scheduling and monitoring patient appointmentsCommunicate regularly with patients through phone or EMR portal regarding visits, medication needs, and/or anticoagulation service proceduresProvides patient education on medication interactions, diet instructions, and adverse reactions related to anticoagulationSupport pharmacists with other administrative tasksIdentify and escalate medication issues that fall outside established protocolsCommunicate effectively with patients, providers, staff members, service teammates, and pharmaciesOptimize anticoagulation service workflows and processes to improve efficiencies and improve outcomesEnsure maintenance of and adherence to established policies and procedures related to anticoagulationMaintain accurate and appropriate documentation within EMRAssist with report development and tracking of service metricsAssist the service, clinical team, regional chief medical officer, and operations partners with ad hoc projects and responsibilities as indicatedOther duties as assigned QualificationsRequired Knowledge, Skills, and Experience Active Certified Pharmacy Technician (CPhT)Current and unrestricted pharmacy license or required training/certification per state guidelines3 years’ experience in a pharmacy technician roleExcellent written and verbal communication skills and demonstrated ability to develop effective lines of communication with all levels of staffAdept at reading, writing, mathematics; multi-tasking; work well within stressful environmentAbility to work independently with minimal supervisionStrong interpersonal, analytical and problem solving skillsPreferred Knowledge, Skills, and Experience Proficiency with Microsoft Office (Word, Excel)Experience working with pharmacy technician protocols 1 or more years working in an anticoagulation serviceExperience with EMR or pharmacy system Posted Min Pay Rate USD $21.70/Hr. Posted Max Pay Rate USD $26.19/Hr.
