Harrison's Hope Hospice is hiring a Hospice Social Worker to support Meridian and surrounding counties. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is requiredDemonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently providedCounsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficultiesProvides accurate and timely documentation of patient services to reflect the plan of careProvides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needsServes as liaison between patients and families/caregivers and community agenciesQualifications:Social Work DegreeIdaho: Bachelor’s degree in Social Work; Master’s degree preferred.Current State Licensure in Social WorkIdaho: LSW, LMSW, or LCSWMinimum of one year’s experience in health care or hospice settingValid driver’s license and proof of insurance is required#ACHOS#IndeedHOSEmployee wellbeing is top priority at Addus Homecare, and we're thrilled to announce our recognition as the top healthcare company on Indeed's 2024 Top 100 Work Wellbeing Index.
