As a Hospice Aide you will: Provide assistance with at least 2 of the following ADL's including personal hygieneAssist patients with medication intake and prescribed exercisesIncidental household services, which are essential to patient health careWill follow the Plan of Care (Instruction Sheet) as instructed and make appropriate updates during scheduled visitsBe responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care QualificationsMust be a certified nurse’s aide (CNA).Must maintain a valid driver's license, good driving record and reliable automobileHigh school graduates or GED preferredDemonstrate the ability to be able to work within a multi-disciplinary team settingDemonstrate the ability to work in field situations Pay Transparency Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
