Key Responsibilities:

  • Lead and manage the Case Management department, ensuring quality service delivery in accordance with hospital policies and accreditation standards.
  • Develop and implement departmental goals and objectives aligned with the hospital’s mission.
  • Oversee staff recruitment, training, and development, fostering a positive workplace culture and ensuring compliance with continuing education requirements.
  • Collaborate with interdisciplinary teams to enhance care coordination and streamline discharge planning.
  • Monitor departmental budgets and performance improvement initiatives, reporting variances and recommending necessary actions.
  • Maintain relationships with community agencies to facilitate patient care post-discharge.

The ideal candidate will possess both clinical and leadership experience in an acute care settings as well as their CCM certification.
