Requirements:
- Current LICSW or LMHC
- 3+ years of professional clinical experience
- Communicate effectively, make critical decisions and demonstrate the knowledge and skills necessary to provide care.
Responsibilities:
- Works directly in treatment relationship with patients and families, individually or in groups
- Ensure service quality and participate in care coordination to ensure proper communication between clinical associates, patients, and referral sources
- Participate in performance improvement activities
- Maintain current practice knowledge
- Investigates case situations and presents information to Physicians and other members of the health team, on patient's family and social background pertinent to diagnosis and treatment
