Seven Healthcare are seeking a dedicated and detail-oriented Clinical Pharmacist to join our healthcare team in Peridot, AZ. The Clinical Pharmacist will play a critical role in patient care by optimizing medication therapy, ensuring the safe and effective use of pharmaceuticals, and providing patient education. This position requires a strong commitment to improving patient outcomes through evidence-based practices, collaboration with healthcare professionals, and direct patient care.

*Responsibilities of the Pharmacist*:
* Review and interpret physician orders and prescriptions for accuracy and appropriateness.
* Develop individualized medication therapy plans based on patient needs, conditions, and treatment goals.
* Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy and achieve desired therapeutic outcomes.
* Monitor patient responses to therapy, identify potential adverse effects, and recommend modifications as necessary.
* Conduct patient education on medication use, side effects, and drug interactions.
* Perform medication reconciliation and review patient records for accuracy, allergies, and potential drug interactions.
* Participate in clinical rounds, case reviews, and interdisciplinary team meetings.
* Maintain compliance with all state, federal, and organizational regulations regarding the use of medications.
* Document clinical activities and interventions in the patient's electronic medical record.
* Stay current with advances in pharmacotherapy and promote best practices in medication management.
* Mentor and train pharmacy staff, interns, and students as needed.

*Applicant Requirements:*
* BLS Certification
* Licensed Pharmacist Certification
* 2 years of Pharmacist experience required
* Acute Care experience required
