This role includes providing education and support to patients, families, staff and caregivers. The goals of this position include improving patient care and flow, eliminating barriers, and improving the overall patient experience by connecting the patient and family with appropriate resources and services. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, safety and financial components; promoting integration of a seamless care model; coordinating patient discharge planning for post-acute services, length of stay (LOS) management and utilization management; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Serves as a leader and resource for the health care team. Integrates new knowledge and evidence-based research into clinical practice. This position reports to the Clinical Nurse Manager and/or the Director of Nursing.Determine the appropriate appointment scheduling, by assessing patient pain, severity of injury, and schedule with appropriate physician group.Responsible for coordinating the care of patients in assigned service line.Knowledge and ability to ask appropriate clinical questions, and illicit patient information to determine appropriate action(s).Priorities include improving patient care and flow, eliminating barriers, and improving the overall patient experience by connecting the patient and family with appropriate resources and services.Access to multiple network clinical information systems to gather and verify patient demographic and clinical information.Collaborate with all care providers to assist in the provision of quality, cost effective patient care.How we will care for you, while you care for our patientsComprehensive benefits package to meet your financial, health, and work/life balance goals.
