Social Worker

As a Social Worker specializing in hospice care, you will provide emotional support and psychosocial evaluations for patients and their families during their end-of-life journey. With a focus on compassionate care, you will work collaboratively with a dedicated team to create meaningful experiences and ensure patients receive the support they need. Ideal candidates possess a Master's degree in social work, a current state license, and have clinical experience in healthcare or hospice settings.

• A heart to serve patients and families and a passion for providing the best possible care
• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
