With expertise in transitions of care, chronic disease management, advanced care planning, and palliative care, this role supports patients throughout the continuum of care. Responsibilities include developing and implementing clinical protocols, evaluating and managing both acute and chronic conditions, and coordinating referrals and hospital follow-ups. The physician also supervises clinical support staff, contributes to interdisciplinary care planning, and maintains detailed, timely documentation in compliance with clinic, state, and federal standards. Active participation in quality assurance, continued professional development, and internal meetings is expected, ensuring best practices in patient-centered care.
