We are looking for experienced Licensed Nurse Practitioners, NPs, General Medicine Nurse Practitioners, for “SNF-ists,” in-house nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting.
Primary Responsibilities:
- Act as a Skilled Nursing Facility’s leading provider of internal medicine services.
- Collaborate with the facility’s interdisciplinary team and physicians.
- Serve as a day-to-day clinical leader while helping oversee the care of all residents.
- Work with the entire facility’s patient population - short and long-term residents.
- Prescribe all necessary medications and treatments.
- Provide monthly stability visits for long term residents.
- Perform comprehensive physical examinations of patients.
- Develop and implement patient management policies and procedures.
- Facilitate referrals to other healthcare professionals and medical facilities.
- Communicate with attending physicians and/or specialists regarding patient care.
- Be comfortable and proficient with documentation and CPT coding.
- Provide evaluations for patients exhibiting changes in condition.
- Advanced care planning.
- Oversee residents day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.
- Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.

Requirements:.
- Master’s Degree Required
- Advanced Practice Licensure (APN) in the state of North Carolina
- Must have a DEA #
- Previous experience in nursing or other medical fields
- Minimum one-year experience in a Skilled Nursing setting (preferred)
- Ability to build rapport with patients and their loved ones.
- Ability to thrive in a fast-paced environment.
