Internal Medicine - Geriatrics Physician at LTC Provider Care, LLC summary: An Internal Medicine - Geriatrics Physician provides primary care services to patients in skilled nursing and long-term care facilities, aiming to improve quality of care, patient outcomes, and facility operations. The physician will serve as a Medical Director overseeing up to three facilities, ensuring medical standards and enhancing patient relationships. This role requires a valid Michigan medical license and DEA license, with responsibilities including commuting between assigned facilities in the Grand Rapids area. LTC Physician Care, PC is a medical group dedicated to serving patients by providing Primary Care (PCP) services to Skilled Nursing and Long-Term Care facilities across Michigan. Our goal is to improve the overall quality of care, patient outcomes, quality of life, refine facility processes, and enhance patient relationships. LTC is seeking a dedicated B/C Physician to join our team as Medical Director Requirements and QualificationsExperience in Family Medicine, Internal Medicine, or Geriatrics is strongly preferred.Candidates must hold a valid and unrestricted medical license in Michigan.
