Host Home / Shared Living Program Supervisor at Mosaic summary:

The Host Home / Shared Living Program Supervisor manages relationships with independent contractors to ensure compliance with state regulations and quality standards in providing services to individuals with intellectual disabilities. This role involves recruiting contractors, monitoring service plans, and facilitating communication between stakeholders while promoting a positive environment. The position requires strong communication skills, operational expertise, and a commitment to improving the lives of those served.

We’re looking for an Independent Contract Program Supervisor to join our team

 

Reporting to the Independent Contract Program Manager, the primary focus of this role is to ensure the individuals served by contractors in home environments are compliant with state rules, regulations and Mosaic’s quality standards and requirements.

What YOU’ll do:

  • Cultivate, build and maintain positive relationships with independent contractors, family members and others stakeholders
  • Recruit new independent contractors to ensure we have enough qualified providers to provide the services needed
  • Monitor service/program plans, assessments, and goals, making certain that the personal outcomes for individuals served are being met
  • Review information from medical visits, providing follow-up to monitor medication administration for accuracy. 
  • Review and provide assistance as needed to develop and implement service plans while ensuring proper documentation

SCHEDULE: Days, salaried-full time, on call responsibilities to support contractor/dept needs, travel required up to 2 hours - vehicle or mileage reimbursement available. Position will office in Grand Island.

What YOU’LL Need:

  • Strong verbal and written communication skills

  • Excellent listening, attention to detail, problem-solving and organizational skills
  • Two years of work experience preferably with people with intellectual disabilities
  • A love of operations and creating seamless, efficient environments
  • Bachelor’s degree in a related field preferred
