Must achieve and maintain national certification in the Nurse Practitioner specialty Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice and/or Palliative background preferred. Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs.
